Waiver & Acknowledgment

1. RELEASE OF LIABILITY

I understand that participation in the German ID Scouting Camp involves physical activity and, as with any sport, there is a risk of injury. I release INNOV8 Global, United In Football (UIF), City Futsal, and all associated coaches, staff, and affiliates from any liability, claims, or demands arising from participation in this camp.

2. MEDICAL CONSENT

I authorize camp staff to seek emergency medical treatment on behalf of my child if I cannot be reached, and I assume all responsibility for any medical costs incurred.

3. MEDIA RELEASE

I give permission for photos and video footage of my child taken during the camp to be used in marketing materials, social media, or promotional content for INNOV8, UIF, or camp sponsors.

4. CODE OF CONDUCT ACKNOWLEDGEMENT

I understand that all participants must follow staff instructions and maintain respectful behavior on and off the field. Disruptive or unsafe conduct may result in dismissal from the camp without refund.

5. PAYMENT & REGISTRATION POLICY

I understand that my registration is not complete until full payment is received. I acknowledge the following refund policy:

All payments for the UIF German ID Camp are non-refundable, except in the case of event cancellation by the organizers.

• If a participant is unable to attend, no refunds will be issued.
• In exceptional cases (e.g., injury, medical emergency), a partial credit toward a future INNOV8 or UIF event may be considered upon written request and documentation, at the discretion of the organizers.
• Camp registration may not be transferred to another individual unless approved in writing by INNOV8/UIF staff.
• No refunds or credits will be granted for no-shows, partial attendance, or dismissal due to behavioral reasons.
